Synriam Tablet 150/750mg is a fixed-dose combination antimalarial medication containing arterolane maleate (150mg) and piperaquine phosphate (750mg). This innovative treatment offers a potent approach to combating malaria caused by Plasmodium falciparum parasites, including in areas with multidrug-resistant strains.

Uses

Synriam is primarily used for the treatment of acute, uncomplicated malaria caused by Plasmodium falciparum in adults and children weighing 20 kg or more. It’s particularly effective in areas with multidrug-resistant strains ,It may also be effective against other Plasmodium species, though its primary indication is for P. falciparum infections.

Benefits

  • Effective against multidrug-resistant strains of P. falciparum
  • Provides rapid parasite clearance
  • Offers a shorter treatment course compared to some other antimalarials
  • Combination therapy reduces the risk of parasite resistance
  • Generally well-tolerated by most patients
  • Convenient once-daily dosing regimen
  • May provide post-treatment protection due to long half-life of piperaquine
  • Suitable for both adults and children (weighing ≥20 kg)

How It Works

Arterolane maleate is a synthetic trioxolane that rapidly kills blood-stage malaria parasites by generating toxic free radicals. Piperaquine phosphate is a bisquinoline that acts by interfering with the parasite’s ability to digest hemoglobin. This dual-action approach provides both rapid parasite clearance and prolonged antimalarial activity.

Dosage

Patient Age Daily Dosage Treatment Duration
Young Adult (20-35 kg) 1 tablet once daily 3 days
Older Adult (>35 kg) 1 tablet once daily 3 days

Take Synriam on an empty stomach, at least 3 hours before or after meals, with a full glass of water.

Side Effects

Common side effects may include headache, nausea, vomiting, abdominal pain, and dizziness. Most side effects are mild and resolve on their own. If you experience severe or persistent side effects, contact your healthcare provider immediately.

Warning

Synriam may cause QT interval prolongation in some patients. It should be used with caution in patients with a history of cardiac arrhythmias or other conditions that may increase the risk of QT prolongation. Avoid using Synriam in patients with severe liver or kidney impairment without close monitoring.

Pregnancy and Breastfeeding

The safety of Synriam during pregnancy and breastfeeding has not been well-established. Pregnant or breastfeeding women should consult their healthcare provider before using this medication. Alternative antimalarial treatments may be recommended in these cases.

Interaction

  • May interact with drugs that prolong the QT interval
  • Potential interaction with strong CYP3A4 inhibitors or inducers
  • May interact with antiretroviral medications
  • Possible interaction with certain antipsychotic drugs
  • Could interact with other antimalarial medications

Precaution

Monitor patients with cardiac conditions, liver or kidney impairment. Confirm malaria diagnosis before treatment when possible. Use caution in elderly patients. If vomiting occurs within 30 minutes of dosing, repeat the dose immediately.

Important Information

Complete the full course of Synriam as prescribed, even if symptoms improve before finishing the medication. This helps ensure complete parasite clearance and reduces the risk of recurrence or drug resistance. If vomiting occurs within 30 minutes of taking a dose, repeat the dose. Store Synriam at room temperature, protected from light and moisture. Keep out of reach of children. While Synriam is effective against malaria, it’s not a substitute for malaria prevention measures such as using insecticide-treated bed nets and mosquito repellents in endemic areas.

    Hello Ifeoma,

This is an overdose situation and needs urgent attention. Synriam is meant to be taken 1 tablet once daily for 3 days, but your brother has taken 3 tablets at once, which is 3 days’ dose in a single intake.

    You should:

    • Seek medical help immediately – take him to the nearest hospital or emergency room.

    • Do not wait for symptoms to show (such as nausea, vomiting, dizziness, irregular heartbeat, or fainting).

    • Take the drug pack with you to the doctor so they know exactly what he took.

👉 Please act quickly — an overdose of Synriam can be dangerous, especially because piperaquine can affect the heart rhythm.
