Gliptus Plus Tablet is a combination antidiabetic medication containing Vildagliptin, a dipeptidyl peptidase-4 (DPP-4) inhibitor, and Metformin, a biguanide. This dual-action tablet effectively controls blood glucose levels in adults with type 2 diabetes mellitus.
Uses
Gliptus Plus Tablet is prescribed for the management of type 2 diabetes mellitus in adult patients whose blood glucose levels are not adequately controlled with metformin alone or who are already being treated with separate tablets of vildagliptin and metformin.
Benefits
- Improved glycemic control through dual mechanisms
- Reduced risk of hypoglycemia compared to sulfonylureas
- Weight-neutral or modest weight reduction effects
- Convenient fixed-dose combination improves adherence
- Effective HbA1c reduction
- Lower fasting and postprandial glucose levels
- Preserves beta-cell function
- Complementary modes of action enhance efficacy
How It Works
Gliptus Plus Tablet functions through complementary mechanisms. Vildagliptin inhibits the DPP-4 enzyme, which increases incretin hormone levels, stimulating insulin secretion and reducing glucagon release. Metformin decreases hepatic glucose production, reduces intestinal glucose absorption, and enhances peripheral glucose uptake and utilization.
Duration of Action
The therapeutic effects of Vildagliptin last approximately 24 hours, while Metformin’s glucose-lowering effect persists for 8-12 hours. The combination provides sustained glycemic control throughout the day when taken as prescribed.
Dosage
Condition | Recommended Dosage | Frequency |
---|---|---|
Initial therapy | 50mg/500mg | Twice daily |
Inadequate control on metformin | 50mg/1000mg | Twice daily |
Switching from combination therapy | Equivalent to current doses | Twice daily |
Warnings
Not suitable for patients with type 1 diabetes or diabetic ketoacidosis. Discontinue temporarily before procedures requiring restricted food intake. Risk of lactic acidosis, particularly in patients with kidney dysfunction. Not recommended for patients with severe hepatic impairment or excessive alcohol consumption.
Side Effects
Common side effects include gastrointestinal disturbances such as nausea, vomiting, diarrhea, abdominal pain, and loss of appetite. Other possible effects include mild headache, dizziness, tremor, and metallic taste. Most side effects are mild to moderate and tend to resolve with continued treatment.
Pregnancy and Breastfeeding
Not recommended during pregnancy or breastfeeding. Alternative insulin therapy is preferred for glycemic control during pregnancy. Women of childbearing potential should use effective contraception during treatment with Gliptus Plus Tablet.
Interactions
- Iodinated contrast agents (increased risk of renal impairment)
- Alcohol (enhanced hypoglycemic effects and lactic acidosis risk)
- Corticosteroids (decreased glucose-lowering effect)
- Diuretics (potential for decreased renal function)
- ACE inhibitors (may enhance hypoglycemic effect)
- Cationic drugs (potential competition for renal tubular transport)
- Beta-blockers (may mask hypoglycemic symptoms)
Precautions
Regular monitoring of renal function before and during treatment. Vitamin B12 levels should be monitored during long-term use due to potential deficiency. Temporarily discontinue in cases of dehydration, severe infection, or hypoxia. Avoid in patients with history of pancreatitis. Exercise caution in elderly patients.
Important Information
Take Gliptus Plus Tablet with meals to reduce gastrointestinal side effects. Regular blood glucose monitoring is essential to evaluate treatment efficacy. Hypoglycemia risk is increased when combined with insulin or insulin secretagogues. Maintain adequate hydration during treatment.
